In business for over sixty years, Blue Cross Blue
Shield of New Jersey is the largest health care provider in New
Jersey. Blue Cross Blue Shield of New Jersey has won many awards,
including an award for having the top ranking in the New Jersey
HMO Performance Report in 2004 and the 2004 Brand Excellence Award
from the Blue Cross Blue Shield Association.
Horizon HMO offers members affordable insurance through
their network of participating physicians. There is no annual deductible
or coinsurance for treatments except for a copayment on prescription
medication. A primary physician oversees your medical care and can
refer you to specialists in the network.
Horizon Basic Plan A/50 allows you to pick any doctor
that you choose with an annual deduction and 50% coinsurance. There
is a cap yearly cap on coinsurance should you or a member of your
family require unexpected serious medical care.
Horizon Traditional Plan B, C, and D
Horizon Traditional Plans are fee for service plans.
You will be able to choose any physician that you like under these
plans. Under Horizon Traditional Plan B, your insurance will pay
60% of the costs after the deductible has been met. Horizon Traditional
Plan C will pay 70% of the costs after your deductible has been
met. Horizon Traditional Plan D will pay 80% of your medical costs
after your deductible has been met.
Horizon Basic and Essential EPO and EPO Plus Plans
offer you the ability to choose any physician or specialist in the
Network without needing authorization from a primary physician.
You may only see a physician who is not a member of the Network
in the case of an emergency.
