Health Net of Oregon Coverage:

HealthNet offers the following options for individuals and families:

• Pearl 25: An HMO plan, Pearl 25 does not require a deductible for most benefits and offers a minimal copay. Patients are covered for office visits, preventative medicine, hospitalization, and prescription drugs (with a one hundred dollar deductible). Patients choose a PCP who provides referrals to necessary specialists.
• Diamond 15 is a PPO plan which allows members to choose a Primary Care Physician who is in-network or non-network, with a 20% copay for the former and a 50% copay for the latter, and which offers a range of premiums ($250 - $7,500) and copay options.
• Emerald 40 PPO: Emerald 40 offers a higher deductible and lower premiums than the Diamond 15 plan.
• Garnet 50% PPO: This plan offers a choice of a $10,000 deductible or of no deductible, and a 50% copay for both in-network and non-network physicians.
• Crystal HDHP: This PPO plan is designed for use with your HSA account. With deductibles ranging between $1,000 and $7,000 (depending on your choice of provider), members have lower premiums.

Health Net of Oregon does their best to offer employers, individuals, and medicare recipients the coverage and the help they need.
