Empire Blue Cross Blue Shield of New York Plans

  • Direct Payment Health Management Organization (HMO) members receive quality service from health care providers in the HMO network. Services covered include preventative and primary health care services, inpatient and outpatient hospital services, and maternity care.
  • Direct Payment HMO/POS members can visit participating health care providers who are in the network at a lower cost while still being able to see providers outside of the network. HMO health care providers will require a small copayment. Members must pay deductibles and coinsurance when using providers outside of the network. Services covered include preventative healthcare, physician, pharmacy, and emergency services. This plan is available to New York State residents between the ages of 18 and 65.
  • Child Health Plus is available to children under the age of 19 who are New York state residents. Costs for this program are determined by the family's gross monthly income. Benefits include preventative health care, emergency care, and prescription drugs coverage.
  • TraditionalPLUS Hospital members receive benefits including outpatient surgical facility coverage, inpatient physical therapy coverage, maternity care in birthing center coverage, and inpatient hospital semi-private room coverage.
