BCBS Arizona Health Plan Services

All of BCBSAZ’s health plans include: doctor office visits, hospital care, chiropractic care, prescription drug, routine physicals and screenings, well-child care, immunizations and mental health services.

Individual Health Plans

These health plans are available for those who do not have health benefits from an employer.  These include:

  • Blue Preferred: (PPO)  Co-pays, and provides a list of providers and five choices of deductibles
  • Blue Preferred Saver: (PPO/HSA Compatible) Has high deductibles with two options
  • Blue Preferred Basic: (PPO) Has a lower premium, and a high deductible with two options
  • Blue Classic: (Indemnity) This is a traditional plan available with six deductible options
  • Blue Classic Saver: (Indemnity/HSA Eligible) Traditional plan with high deductible
  • Blue Select: (HMO) Has 2 options
  • Blue Select Plan 2:  A co-pay is required for most services
  • Blue Select Plan 3:  Has a lower premium, deductible and coinsurance apply to inpatient charges

Group Plans

Group plans offer many flexible options like PPO, PPO upgrade options, HMO, dual (PPO and HMO), dental and consumer-directed plans.
