Affinity Health Plan is a non-profit managed care company that provides health care services for over 200,000 low and moderate income residents around the New York City area.  Since its beginning in 1986 as the Bronx Health Plan, the Affinity Health Plan’s dedication to improving the public sector has set a standard for the development of the State Medicaid care program and subsidized programs for the uninsured.

Affinity Health Plan has created a solid foundation to positively change the community they serve.  By promoting health and emphasizing the importance of prevention, it has contributed to the betterment of countless lives, providing its members with a network of 5,000 specialists and 1,400 primary care providers in over 60 hospitals.

Affinity Coverage

The Affinity Health Plan offers services to families and seniors in New York City, Suffolk, Worcester, Nassau, Orange, and Rockland Counties.  Coverage is offered through the following options:

  • Family Health Plus:  for low-income, uninsured residents who do not qualify for Medicare or Medicaid.
  • Child Health Plus:  designed for children 19 years old or younger, who are uninsured and do not qualify for Medicare or Medicaid.
  • Medicaid Managed Care:  for Medicaid beneficiaries who are eligible through the Transitional Medical Assistance, Temporary Assistance to Needy Families, or Supplemental Security Income programs. 
  • Medicare Advantage:  designed for residents of the New York City area who do not qualify for Medicaid or Medicare government payment of Medicare cost-sharing.
  • UniCare:  uninsured individuals and families already in an Affinity Health Plan may choose this option, where a portion of the premium is paid by the state, with the remaining premium and co-payments paid by the member.
